  • DREAM Program

    In December 2023, The Alzheimer Society Waterloo Wellington and Grand River Hospital launched the Dementia, Resource, Education, Advocacy, Mentorship (DREAM) program.

    The Dementia Resource Consultant works in the Emergency Department as part of the Integrated Dementia Resource team (DREAM Team). The team aims to intervene early and support people living with dementia to go safely home with service coordination and collaboration with ED staff, introduction to Alzheimer Society Waterloo Wellington services and support, linkages to community partner organizations, along with reassurance, information, activities and access to respite to reduce hospital admissions and support going home.

    The onset of his dementia brought a dramatic change to the relationship Hannah has with her father. Dementia, Dad, and Me follows the two of them as they find new ways to connect, and to navigate the delicate transition of their roles from father and daughter, to caregiver and cared for.
